Ann Arbor, MI – The Center for Information Management, Inc., an Ann Arbor, MI-based provider of information systems and services for organizations caring for older adults, has recently deployed a comprehensive Nursing Facility Transition (NFT) Portal in conjunction with staff at the Michigan Department of Community Health. The web-based application was designed and developed by CIM Inc. and provided as centrally-hosted solution to all agencies providing Home and Community Based Waiver Services in Michigan to more closely coordinate business operations between local transition workers and State program managers. The web-based NFT Portal allows all agencies to report transition activities, services and outcomes, and associated supports required reporting to the Centers for Medicare and Medicaid Services regarding participants in the Money Follows the Person (MFP) initiative.
